3/9/2024 EnchantaVaughn Approved Behavior: Upon arrival to the home setting, Laylah was in a composed mood, as evidenced by her comfortable demeanor. QMHP-C engaged her in a discussion to determine how her day was going. She verbalized to QMHP-C her day was going okay. Mother verbalized to QMHP-C on how Laylah will get upset with her and isolate herself when things do not go her way with behaviors lasting up to 10 minutes in duration. Intervention: QMHP-C educated Laylah about using good manners at home and offering alternative methods and suggestions to model appropriate language that Laylah should use to ask for things or to get someoneâs attention. QMHP-C processed Laylah feelings and offered choices of what to do when faced with conflict or challenged in the home setting. QMHP-C facilitated a family discussion to talk about common mistakes children with ADHD make and how she can make better choices. QMHP-C reviewed home expectations and how Laylah should conduct herself around peers and authority. QMHP-C praised and recognized Laylah for her hard work, efforts, participation, and working towards improvement by changing daily habits and implementing a positive and consistent routine. QMHP-C supported Laylah with meeting her objectives and coping with stressors without needing additional redirection or prompts. QMHP-C encouraged Laylah to continue displaying age appropriate behavior, and the benefits of this will be reviewed with parent. QMHP-C conducted a review of previous week and asked Laylah what she learned from it. QMHP-C assisted Laylah with problem-solving. QMHP-C provided Laylah with positive feedback. QMHP-C provided Laylah with positive reinforcement. QMHP-C encouraged Laylah to practice relaxation and coping techniques in social/virtual environment, at home, and in the community. Response: Laylah utilized good manners, such as reframing from cussing and street terms and talking on her age level. Laylah displays fair feelings in the home. Laylah recognized her triggers and was able to identify common interests and similarities she and other ADHD recipients shared. Laylah understands that her actions have consequences. Laylah is striving for a good report from mom and counselor every day. Laylah wants to make her counselor and parents proud by doing a good job every day. Laylah retained a lot of the information she was taught which motivates her to pay more attention and take her time when going into the community especially with everything thatâs going on. Laylah is observed problem solving on her own and handling her emotions appropriately. Laylah avoids meltdowns and lashing out when upset or frustrated in regard to school ending for the rest of the year and leaving some of her possessions at school. Laylah responds well to feedback. Laylah responds well to the positive reinforcement provided by QMHP-C and is seen increasing self-esteem and confidence. Laylah said she was going to practice coping strategies and relaxation techniques in all aspects of her daily living routine in hopes that she will be a better person and be able to stay calm and stay motivated.
